Questo caso descrive la gestione di un atleta di calcio di 14 anni con osteocondrite dissecante del condilo femorale laterale e una successiva lesione condrale di grado IV nella troclea femorale. Il trattamento iniziale con trapianto di osteocondrale fresco ha portato all'osteointegrazione, alla risoluzione del dolore e al ripristino della funzionalit\u00e0. Durante la riabilitazione, si \u00e8 sviluppata una lesione trocleare secondaria, che \u00e8 stata trattata con una combinazione di tessuto adiposo autologo microframmentato e una membrana di collagene.
