Ce rapport de cas d\u00e9crit la prise en charge d'un athl\u00e8te de football de 14 ans atteint d'ost\u00e9ochondrite diss\u00e9quante du condyle f\u00e9moral lat\u00e9ral et d'une l\u00e9sion chondrale de grade IV dans la trochl\u00e9e f\u00e9morale. Le traitement initial par transplantation d'ost\u00e9ochondre frais a permis une ost\u00e9oint\u00e9gration, une disparition de la douleur et une restauration de la fonctionnalit\u00e9. Au cours de la r\u00e9\u00e9ducation, une l\u00e9sion trochl\u00e9enne secondaire s'est d\u00e9velopp\u00e9e et a \u00e9t\u00e9 trait\u00e9e avec une combinaison de tissu adipeux autologue micro-fragment\u00e9 et d'une membrane de collag\u00e8ne.
